ANNOUNCEMENT: TO ALL INCOMING FRESHMEN and TRANSFEREE STUDENTS
PRE-REGISTRATION PROCEDURE
The following documents will be required for the Pre-registration starting on July 18, 2022:
CSU College Admission Test (CAT) Result* (3 copies)
1. Accomplished Student Profile** with 2x2 ID Picture (2 original copies)
2. Additional 2x2 ID Picture (2 copies)
3. Grade 12/SHS Card (Form 138)*** (1 original and 2 photocopies)
For Transferee Students (Honorable Dismissal and Copy of Grades)
4. PSA Birth Certificate (3 photocopies)
5. Certificate of Good Moral (1 original and 2 photocopies)
6. Vaccination Card (2 photocopies)
7. Proof of Income (BIR ITR/Indigency/4Ps/etc.) (1 original and 2 photocopies)
8. Place these documents in 2 long brown envelopes with your full name (last name, given name, middle initial) written at the back of the envelope.
9. *Priority will be given for those who met the Percentile requirements per College:
College of Information and Computing Sciences = 20 and above
College of Hospitality Management = 20 and above
College of Business Entrepreneurship and Accountancy =20 and above
College of Teacher Education = 60 and above
College of Criminal Justice Education = 60 and above
College of Agriculture = 60 and above
